SNF PDPM Basics and Impacts in Care Delivery Partners Toolkit

Follow these three easy steps below! 

Step 1	Click the orange log in button in the upper left hand corner. Use your user name and password that you would use for LTC Trend Tracker or the AHCA NCAL website.   Don’t have an account or can’t remember? Click the NEED HELP button and the next button that reads-“Are you new to the site? Create an Account”. *Please note* you must add your member facility name to your login Once you are in and can see your name in the upper left hand corner, you are ready to go!
Step 2	From the home page click on a category (Resources, Training, Webinars or On-Demand Training) For this demonstration, let’s try Webinars- Upcoming Live.  On the “upcoming webinars” page go to a product in which you are interested. Click the green REGISTER button. Or click the title of the webinar and you will be redirected to the page where you can view the overview, handouts or resources associated with that product. You can also click REGISTER in the upper right hand corner. The content will be available in your dashboard.
Step 3	Almost there! If the resource you are trying to access is a paid-for product, fill out the registration details and voila! The products are yours to use and can be found in your dashboard. Need further help? Email us at educate@ahca.org We’ll walk you through it.

Because CMS has provided limited to no guidance to SNF care delivery partners impacted by the PDPM implementation on October 1, AHCA members have asked if we could create a webinar about PDPM that providers could share with referring hospitals, physicians, Medicare Advantage plans, Accountable Care Organizations, and Bundled Payment program partners.

To help facilitate AHCA member discussions with care delivery partners about PDPM, we have prepared a one page document that includes links to the following five pre-recorded webinars and slides that are intended to be viewed by SNF care delivery partner to get a better understanding about what PDPM is, and how PDPM may impact their relationships with SNF providers. No password is necessary to access this content. 

Each webinar recording (approximately 30 minutes each) contains the following content:

• An overview of the PDPM payment model
• Key clinical characteristics impacting PDPM
• Specific PDPM considerations for SNF partners, and (each recording has partner-specific content)
• Where SNF care delivery partners can go for more information

Each recording is identical except for the third item that includes content that is customized to the specific type of SNF care delivery partner.  AHCA Members are encouraged to open a dialogue with their care delivery partners as part of their PDPM transition preparations.  We also encourage SNFs to consider sharing the respective links below with these partners as a supplement to your discussions.
