Turning Data and Systems into Person-Centered Care

Long term care and post-acute care organizations are put to the test every single day. Consumers, referral sources, and government agencies demand quality care.  Simply stating that you provide person-centered quality care is no longer enough.  You must prove it.  Providing reliable, person-centered care requires data and systems to be in place.  Data drives quality improvement.  Having the right systems ensures your teams are providing reliable care consistently.  This training engages participants in turning data and systems into person-centered care.  It identifies how using Long Term Care Trend Tracker and the Quality Award journey can springboard your center into a best-of-the-best organization.

Objectives: After viewing this training, participants will be able to:

    Describe what person-centered care means.

Explain how the Quality Award journey leads to better outcomes.

Discuss how to use Long Term Care Trend Tracker for quality improvement efforts.

Describe the six core practices for providing reliable, person-centered care.
