Preparing for TEAM: What SNF and PAC Providers Need to Know

Beginning January 1, 2026, the Transforming Episode Accountability Model (TEAM) will introduce a mandatory bundled payment approach for hospitals in selected Core Based Statistical Areas (CBSAs). Under TEAM, hospitals are held accountable for the cost and quality of care during 30-day episodes following five surgical procedures.

Although hospitals are the only formal participants in TEAM, SNFs and post-acute care (PAC) providers can participate as TEAM collaborators -- partnering with hospitals to improve care coordination and patient outcomes.

This session will cover:
  • TEAM’s structure, qualifying episodes, and financial risk tracks
  • How to evaluate the potential scope and impact of TEAM using LTC Trend Tracker℠
  • Assessing your facility’s readiness using the TEAM Preparedness Checklist
