Beginning January 1, 2026, the Transforming Episode Accountability Model (TEAM) will introduce a mandatory bundled payment approach for hospitals in selected Core Based Statistical Areas (CBSAs). Under TEAM, hospitals are held accountable for the cost and quality of care during 30-day episodes following five surgical procedures.
Although hospitals are the only formal participants in TEAM, SNFs and post-acute care (PAC) providers can participate as TEAM collaborators -- partnering with hospitals to improve care coordination and patient outcomes.
This session will cover:
- TEAM’s structure, qualifying episodes, and financial risk tracks
- How to evaluate the potential scope and impact of TEAM using LTC Trend Tracker℠
- Assessing your facility’s readiness using the TEAM Preparedness Checklist
Speakers:
- Rohini Achal, Population Health Policy Analyst (AHCA)
- Krista Virost, Sr. Director, Value Based Contracting (AHCA)
- Terry Hawk, Sr. Research Analyst (AHCA)
Registration is free to AHCA/NCAL members. If you are not able to attend live, the recording will be provided to you.
